Religious Objection to Immunization/Physical Examination: Under Illinois law, parents or legal guardians who object to immunizations and/or health, dental, or eye examinations, or any part thereof, on religious grounds are not required to submit their children to the examinations or immunizations. When a religious exemption is requested, the parent(s) or legal guardian(s) must complete the Certificate of Religious Exemption form provided by the Illinois Department of Education. The form must be signed by the child’s parent or legal guardian AND the child’s health care provider (includes physicians licensed to practice medicine in all of its branches, advanced practice nurses, or physician assistants) responsible for performing the child’s health examination.

CPR and AED Video Resource for Staff and Families

State law requires the Illinois High School Association (IHSA) to post a hands-only cardiopulmonary resuscitation and automated external defibrillators training video on its website. The law also requires the District to notify staff members and parents/guardians about the video. Annual Notice of Integrated Pest Management Plan
Roselle School District 12 is committed to providing a safe environment for the community, staff, and students in our care. We seek to limit exposure to pests and pesticides and, therefore, have adopted the State of Illinois Integrated Pest Management (IPM) approach to pest control and compliance with the Illinois Lawn Care Application and Notice Act. The IPM approach minimizes the exposure to our community, staff, and students to pesticides and includes a variety of non-chemical and chemical methods to prevent and eradicate pests. While pesticides may be used to remediate infestations of pests that may be found in the facility and its surrounding grounds, only the least toxic products will be considered and combined with non-chemical methods whenever possible. At the present time, District 12 does NOT apply pesticides inside the buildings on a regular basis. Roselle School District 12 assumes the responsibility of notifying our community, staff, and students of any upcoming pest and pesticide treatments. The Illinois Structural Pest Control Act, the Illinois Child Care Act, and the Illinois Lawn Care Products and Notice Act require prior notification to requestors when a pesticide application is scheduled to occur.To those who wish to receive said notification prior to any application, please notify Dr. Mary Henderson, Superintendent, by email at: mhenderson@sd12.org or by regular mail to: Roselle School District 12, Attn: Dr. Mary Henderson, 100 E Walnut St. Roselle, IL 60172All applicable rules and regulations regarding notification will be adhered to.
